What types of request can I make through the online verification request form?
You can use the online form to make requests for a digital copy of your certificate and/or verification to be sent directly to a board, agency or other entity such as a school or employer. Multiple requests can be made on one form by checking all applicable options. If you are requesting that we send verification to a state board, don't forget to tell us which state in the memo field. We are no longer offering printed certificates, but if you request a digital certificate, you are able to print your certificate if you would like.
Name Change requests: When completing a name change, please check your profile to be sure your other information is up-to-date and then make your name change request using the form. Be sure to upload the required documents (e.g. legal papers for name changes) to avoid delays in processing your request.
When should I submit my request for verification?
Verification requests should be submitted after you have been notified that you have passed the full CNT course, including both the written and practical exams.
What is the cost per request?
CNT Verifications are free.
How long will it take for my request to be processed?
Due to the large volume of requests, it can take up to 10 business days from the date that a request is received to complete the processing. Please complete all sections of the request form completely to avoid delays.
Can I request that my certificate or verification be expedited?
While there is no expedited service, please contact support@ccahm if you have a specific deadline.
How will I know when my request has been processed?
You will receive an email confirmation when we've received your request. Another email will be sent to you when your request has been processed. Please note we are only able to send verifications to the NCCAOM if you have an active application on file with them.
How can I be sure that the agency received my verification or certificate?
It is highly advisable that you check with the NCCAOM, college, agency or state board for confirmation that your verification or certificate was received. Be aware that the email received from the Council's national office is confirmation that the request was processed and sent and is not a confirmation of delivery.
